H1B Approved but Extension of Stay Denied Need Information

My H1B petition got approved until 2019 but my extension of stay got denied because I exceeded the CPT limit when I was in F1. Currently I am out of status because my F1 got terminated by USCIS
A. I can't go for H1B stamping because I changed the job, Amendment has to be filed.
B. I am thinking to go for H4 stamping immediately since I have no status If I file for H4 It may take several months also there might be a rfe for what status did you filed H4?
1. If I go for H4 stamping once I come back can I change my status back to H1 with new amendment?
2. My Husband changed status from F1 to H1B and H1 B is valid till 2019 with I-94 I see that in many H4 visa questions "When did your husband got stamped?" He didn't get his stamping done after change of status. How should I answer this question?
3. Is there a possibility if VO ask about my previous masters or H1B approval or about my out of status?
