Stamping in India

Hi,
I am planning to visit India and will take appointment in Delhi.
I have a question on DS 157 form, q 12. List last two employers.
I came here through one consultant A, but he failed to place me and after 3 months I got an offer another company B. Past 7 years I am working this company B.
If I mention company A in list of last two employers will it give any problem.
Except this everything is fine. Please advise.

When you not work with company A,Not neccessary to mention in D157...

i.e list of two employers you work with...

My first visa stamp with Company A. I stayed my relatives house for 3 months to get an offer with company B. That year I have w-2 form for company B.
I am not mention in DS 157 is there any problem.
Is there any possiable way to ask w-2 form company A.
