Regarding 221(g) issued for H4. -- Need Urgent help

my wife applied for H4 visa in chennai consulate & they issue 221 G Blue form and told her to send " Permission Leter from Husband giving Permission to wife to carry childeren while travelling US" and Forms DS156, DS157, DS158

Is DS158 is Required for H-4 VISA ?.. I think this form is for J1 Visa's

Please Advise and let us know..

My wife got 221G form for H4 Visa & asked to submit thru' Drop box following Doc's.

1. DS156, DS157, DS158
2. Letter from husband giving permission to carry kids along with her while traveliing to USA

is the Above forms are default for all Blue 221G forms ? I am kind of Confuse is DS158 is required for H4 Visa aswell?

i guess. u can call vfs and consulate and enquire about it.. otherwise u can take the help of some attorney
