Query regarding Surname change in passport

Hi all,
I need an urgent help.
My name is Anita Esther Ketzia.
In my passport i had given it as the same with surname as blank.
I had applied for H1B through my company and now the problem is that they want me to mention my surname as "Ketzia".
Will it be possible to change it that way?
If so, what are the documents that I would need to submit to them.

Please help me out in this regard.

you don't have to change in passport, just provide your SUR NAME to your employer and use it as the LAST NAME, you should be fine.
