H1b actual 6yrs time period..

Hi! Everyone,
First of all, Thanks to all who spend their precious time to answer all possible queries posted out here.

I have 2 different queries. I would appreciate if someone could send me some info on them:

1. How does the SIX years time period being calculated for H1B?
a. From the day a person arrive in USA? OR
b. From the day, the visa has been issued? OR
c. From the day, H1B got approved? OR ANY OTHER WAY?

2. My daughter is on H4. She has a naming error in her passport and I am planning to take that to Indian Consulate , NYC to make it corrected. When I go for stamping next time in India,, will this name correction in the passport create any problem.?

Just adding something more...

Any time spent outside the U.S. (even vacation time) can be re-captured in the 6 yr H1 time. So only the time you are in the U.S. on H1 counts towards the six yrs.
