I would say, write the same which is on your I-797 Approval notice.
In my case, there was a additional space in my middle name in passport.
but on I-797 it was right & I mentioned the same, whatever was there in I-797,in DS-156 and its done.

Hi,

In my passport my full name(i.e FirstName LastName) is printed in surname field and given name is blank.

So in DS-156 form what should I write in surname and first name fields?
