H1 transfer questions

Hi Sir/Madam,
Have few questions in H1B VISA Transfer and traveling to US.

Got H1B Petition Approved/Stamped in December 2008 thru Company A and H1B VISA is valid till 2011. And I have not traveled to US with H1B VISA thru Company A. Right now am working for Company B.

Questions:
1. Is it possible for me to transfer the H1B VISA to Company B, as Company B is interested in sending me to US???
2. What are all the documents required from Company A to travel to US??? Also is it possible to carry Company A H1B VISA and Company B documents with me to Travel to US???
3. As I have not traveled once to US with H1B VISA thru Company A, I donʼt have SSN, Salary Stubs. Is it still possible for H1B Transfer???

Can you please help me to by answering the above questions???
