H1B cases reciepted in error?

There is a post on AILA that some H1B cases were recipted in error. Is this true? I mean How can USCIS make same mistakes every year. This is total BS. Experts Plz provide insight into this issue.
Also, there is possiblity that they are receipted since the quota did not get filled up on first day, isnt it?

What is there to respond? It is credible info and they will correct it.
