L1B to H4 Please help me

My Status & Situation

I am on H1B Valid till 09/30/2009 (Currently in Visa extention process).
My Wife is currently on L1-B which is valid till 10/10/2009.
After that she needs H-4 Visa (As my dependent) effective from 10/10/2009.
She is completing her 5 years continues stay in USA on L1-B.



Now I have following questions,

1. When will be the best time (what date) to apply H4 for my wife?
2. Since my visa extension is in processing stage and by
10/10/2009 if we are not getting approval for my H1 and my wife H4, what is the status of my wife?
3. Can she stay here with me depending on H4 (/H1) petition receipt notice?
4. Is it better to file my H1 Extension & H4 together or it doesn't matter?
5. Do you advice me to go for premium processing?

Filing H1 and H4 together is the best option as the expiry dates are close enough. She cannot work past her expiration date but can remain in US on a H4 pending receipt notice. Going with premium processing will impact her L1 use if it gets approved before 10/10/09.
