H1- First time stamping in Montreal

Hope you are doing well. I have a quick question. I completed my Masters Degree in US and working full time with a shipping corporation. Job is related to what I studied in Masters Degree( International Business). I got the interview appointment confirmed in Montreal. I believe we should submit DS160 form for this location. I would like to know if we have to take DS 156 and DS 157 for the interview. Kindly help. Thanks

DS 156 and 157 are mandatory for any visas for all applicants meeting the age criteria.

I apologize for bothering you. I have a quick Q. I got the unemployment wage reports for 3 quarters(2009) from my company. Is that sufficient for the stamping docs. Also, Is employer's office lease document mandatory? Thanks for the help.
