Urgent! H1B: I94 missing I797

Hi everyone,
Please help me out here.
My father has renewed her H1B the second time (Initial aprroval-->1st extension-->2nd extension). He needs to leave U.S for an emergency,while we're preparing the documents he needs to get visa stamp, we noticed that I797 is missing. The only thing we have is the I-94 slip which is cut off from the I-797 original. And that was the only thing we received from our attorney--the I94 slip. Has this happened to anyone?

Well, talk to the employer/attorney and get the original approval notice. It is required for the visa stamping as well as at the POE.
