H4 visa error: urgent help needed

Recently my wife got her H4 visa stamped (I have shared her visa experience in another thread). However there is an error on her H4 visa, that is:

Her passport correctly has:
sirname: YYY
given name: AAA

Her previous visa F2 also had correct info as:
sirname: YYY
given name: AAA

However in her recent H4 visa, they have put following:
sirname: AAA YYY
given name: FNU

That does not seem correct as her sirname is correctly mentioned on her passport and is not blank.

I would appreciate any info regarding
1. what should I do?
2. How long may it take to correct it?
3. If not corrected, will it create any problem at the port of entry or in the future?

Any other suggestion is welcome.

>>> Correct it. It could be because you filled the DS-160 application wrongly.

>>> Hard to predict. Check the VFS helpdesk.

3. If not corrected, will it create any problem at the port of entry or in the future?
>>> Should not be a big issue since the spellings are the same. Just the first name and last name is been combined. However it is better to get this corrected, since all the documents in U.S (ITIN number, driving license etc) will go by your visa (Will help as proof of identity).
