H1b extension denied-I 140 approved- I94 still valid

Hello,
My H1B extension got denied after 6 years. I have approved I140-Eb2 catagory. My I94 is still valid. I don't want to change employer. Employer is willing to support my situation Following are my questions
1) Can we file MTR? Does MTR has premium processing?How long does regular processing takes?
2) I have heard , I am eligible for cap exempt fresh H1b. Current month is July, Can I apply for new H1b?
3) Can I go back to India before I94 expiration and come back on B1 visa?
4) Do I have L1 visa option?
5) Anyother suggestion?
