Employer information in DS160 for H1B stamping

Hi All,

We planning to travel to India and we will have get our visa stamped on existing approval.
In DS160 Form we need to fill Employer and Employer address

1. Which address are we suppose to fill if a person is working at a client location ? Should i enter the address according to Form I-129 or LCA ?
2. For previous employer - we had an extension in between with the same employer but different work location . Should we have two separate entries for each petition ?
