Visa Screening for physical therapists

Hello,

My wife is a physical therapist from India and intends to obtain a license to practice in the State of Texas. The Texas Board of Physical Therapists requires the following for licesure: (1) Educational credentialing (2)TOEFL/TSE/TWE scores; and (3) Passing score in the National Physical Therapy Exam. Once she obtains all the three requirements state herein, would she still need to obtain a visa screening certificate in order to obtain an H1 visa for healthcare workers? The Texas Board has told us that they do not want the visa screening certificate. Some states do ask for the visa screening certificate for licensure, but Texas doesn't. But the question is, will USCIS issue a visa if she gets a license from Texas?

If there are state specific requriements in some states, those are not federal requirements and USCIS goes only by federal law.
