H1B Extension - Requirements

Hi,

Being in India, I filed for H1B through Company-A and got it stamped.

Later again being in India I transfered it to Company-B and I got a approval notice for the same but never used it. After sometime I left the company-B and I hope they might revoked that approval notice.

Now I am working in US through Company-A's H1B through a vendor company-C at end client and I am planning to file for H1B extension.


My questions are:

1) what are the required documents to file for h1b extension?
2) do i need to submit approval notice of company-B while filing for extension?
3) do i need to submit my experience letters from India?
4) do i need to submit any contract doc between vendor company-c and client?
5) do i need to get sow from vendor company-c?
