How many days does it take for COS from H4 to H1B?
Only after obtaining new I-94 of H1B can one work or after any stage in COS process?

With normal process 2-3 months, premium process 15 days.
You need to wait for getting the approval in order to start working. Also, you cannot work before the 'valid from' date on that approval.

I did not work on my H1B and it is expiring in 3 months, but if I have to work in US for the same company from whom I got H1B, should they have to file COS and H1B extension at once?
