H1 Visa CAP

Latest Update published by USCIS mentions that the H1 Visa capo of 65000 is expected to be reached by December first Week. Could you please let me know what counts for the H1 visa CAP?Is it the Number of VISA stampings done or it is the number of petition approved at USCIS.

Number of new petitions (when the beneficiary never had an H1 petition before or when petition was specifically filed under new cap after 1 year stay abroad).
49200 have been used out of 65000 as of last count. Nowhere does it state that it will reach by Dec.
