My H1B visa has been approved

My employer has applied H1B visa for me and it has been approved and I have received all the documents from them such as I-797, copy of I-129 petition, etc., I have following doubts. Please clarify.

1) My employer paid all fees for my visa. Do I need to pay any charges (Thru HDFC) other than the visa fees? If yes, How much i need to pay?

2) Do I need to fill DS-160 form to get appointment for interview?

3) How to get appointment for interview (Any link)?
