pls help me, H1 extension ? forms to be filled !!!!!!!!!!

Hi,

Paid HDFC charges.

Do I need to fill all the forms as done for 1st time H1-B applied or only DS-160 form I need to fill online !!!!!!

Please guide me what r the forms I need to fill for 2nd H1-B, I mean for extension.

Right now I'm in India, need to book my slot for Visa interview.

Please help me.
