help needed in completing I-129, thanks

Hi I am applying for a change of status from J1 to H1 through a waiver position. I wanted to ask, what do I write in Part 4 (Processing information) question 4. "Are applications for replacement/initial I-94s being filed with this petition?
Is a new I-94 automatically issued with a change of status or do we have to send separate form for that too?

you are not replacing your I-94 so the answer is NO.

I-94 is issued everytime your application is approved, it is an arrival/departure card that you clip in your passport so you can re-enter US if you leave this country together with the stamped visa on your passport. I hope this helps. I did my own H1b application so Im sure my answer is correct.
