Sample Employment Letter - Self Employed

Hi,
Can some one please provide me a sample employment letter for self employed. (ex. Providing IT consulting services, through own company)

Also on Affidavit of support I-134, Column 7, Which company should you specify, Own Company or the client name?

My parents have their appointment at Chennai consulate in Mid may, I need to send them these doc. ASAP
