About USA Tourist Visa

I have some questions regarding USA Tourist Visa
I am a US citizen and my mother is planning to come to the USA in May this year.
She came to USA once before in May 2000 on a 6 month visa. The questions that I have are as follows:
1) Do I have to send the Sponser(Inviter) documents again this time?
2) Her passport bears an old address, does it matter? Should she get the address changed on the passport or is there a provision for a new address on the forms?
We have started the procedure by getting a tracking number from HDFC bank. Please reply soon
