Pl help for requirements forUSA, B2 VISA RENEWAL AFTER 10 YEARS VISA EXPIRY

My uncle's USA visa is for 10 years & expiring within 6 months. There is a planning to visit USA in another 4 months.
required clarifications for
1) How many months early we can apply for new visa or only after the exiry date.
2) whether we require to submit sponsorers forms again for this renewal.
3) In which website , we can get total information of the needy forms to submit for renewal of USA Visa.
