Parents have different name on their passport and different on my passport

  • Parents have different name on their passport and different on my passport

    My Parents does not have a last name on their passport but have it on my passport. They had the appointment yesterday and the counsellor has asked for "Name change document copy".
    I want to confirm whether the notarized affidavit will work for this or not

        Yes. that should work. But here is my question:
        1. What visa did they apply for? usually, counsellor does not care what is parents name on your passport.
        2. Keeping affidavit handy is not a bad idea to backup your demonstration for relationship
