Visitor Visa Extension Checklist - Help

Hello all,
I want to extend the Visitor Visa of my father. He has been staying for 4 months, now want it for 6 more months.

Please let me know if this is the correct list of docs I have to attach:
1. I-539 form.
2. An application stating the reason for the requested extension. (??? Is this just a Word Document where I give the reasons for the Extended stay?)
3. Bank Statement.
4. Copy of your return tickets.
5. Extension fee $140

Do I have to attach the I-134 document too?
and others such as my H1-B, my Tax forms and pay-stubs?

Any help appreciated.

Copy of I-94 also.
Yes, attach new I-134 again.

Don't have to send paystubs, H1 papers and all that.
