If she gets sick, you have to pay the deductible first once. You can choose the deductible such as $100, $250, $500 etc. After you pay the deductible, insurance company pays 80% for first $5,000 expense, you pay 20%. After $5,000, insurance company pays 100% up to chosen policy maximum such as $50,000, $100,000, $500,000, $1,000,000.

She will be here for 6 months.
I will buy as soon as the plans are finalized.

How about the PPO co-payment? How do I need to pay?

As a broker, we have many different insurance plans from several companies.

Currently, the best insurance plan is Protection America - IMG. It has large PPO called First Health and therefore it is widely accepted. If your mother is not sure how she is going to stay, you can renew the plan for as little as 5 days a time. If she leaves early, you can get refund too.

Please look at the brochure at https://www.insubuy.com/patriot-america-insurance/medical-insurance/brochure.jsp
You can make instant purchase right there. You require date of birth and passport number.
