Network negotiated fees for pre-existing conditions

Could you please tell me that when I buy insurance for my parents and if they need to use the medical treatment for a pre-existing condition, will the amount charged by the doctor’s office be discounted or the regular price? I understand that the insurance will not cover the pre-existing condition, but I am wondering if the discount still applies?

Another question – My parents are starting on Thursday morning India time for Chicago, if I buy the insurance online today from buyamericalinsurance and pay the premium using the credit card, will the coverage start before they start?

Also, is the premium non-refundable i.e. if we use only 1 month and had paid for 2 months then the extra month premium is lost?
NARENDRA>>> As long as there are no claims filed from day 1, insurance company will refund the fully unused months minus $25 cancellation fee. By unused months, I mean early departure. If you buy for 2 months, they stay for 2 months and you don't file any claims, there is no refund.
