Pre-existing conditions and routine physical exams in visitors insurance

I was going through the insurence products, but did not find any product covering the pre-existing conditions and routine physical examination if required, I am not sure whether i am missing somethings. So, please let me know of any product which will cover above 2 types of conditions, in insurance plan.

There is no product from any company anywhere that would cover pre-existing conditions or routine physical exams for visitors to USA.
