Switching or upgrading visitors insurance

I have couple of quick questions for Visitors Insurance for my parents arriving in US in April'07 and leaving in July'07.

1. Can I buy IMG Fixed insurance plan to start with and then when needed switch to Comprehensive insurance plan? Is there any catch in switching?

2. What are the coverage limitations in IMG Fixed Insurance?

1. There is no concept of switching or upgrading. You will have to buy a new policy all over again. And if the person had already any problem before the effective date of the new policy, that would be considered as pre-existing condition in the new policy and not covered. Therefore, there is practically no use of buying new policy after getting sick.

Since my parents are healthy and doesn't have any serious pre-existing conditions and also since they will stay in the US for only 3 months I was thinking of getting IMG Fixed Insurance Plan....Do you think this would be the right purchase?

Dad's age is 65 and Mom's 64.

I understand that comprehensive medical coverage would be best fit for my parents. I have few more questions below before I buy one of them.

1. Which is the best comprehensive insurance policy out of the 7 insurance policies?

2. Do I have pay the deductible for $250(if I choose that) everytime I visit the doctors or is it just one time? If I have pay the deductible $250 everytime, do I also pay 20% of the total expenses? Can you please explain with few examples?

3. How do the claim thing works in the comprehensive insurance? When I visit the doctors, do I just show the Insurance card and pay the 20% and the rest is taken care between the doctor and the insurance company or do I have to fill the claim application form everytime?

If I buy Protection America Comprehensive Insurance for my parents, will the doctor visit be covered. For ex- Once they arrive here in the USA and then get fever or common cold and I see my local doctor, will the insurance pay the doctor's fee for such office visit?
