That is relevant only in HMO. There are no HMO plans for visitors insurance.
Protection America is a PPO where you can directly visit a specialist. Therefore, you don't have to choose any PCP.

IMG does not sell directly but only through agents like us.

And many of their products are not general release products. In other words, they are tailored to specific market and sold only through few brokers like us. Those products are not described on IMG's web site. But they are genuine products from IMG.

You don't have to wait for physical card. You can use the virtual id card that you get in the email. However, please keep in mind that insurance company will pay for new medical conditions, injuries or accidents that may occur after the effective date of the policy. It will not cover any pre-existing conditions.

Thanks for prompt reply. So can it be extended in future as i am planning to
take the insurance for only 87 days.If my parents stays more than 87 days then i will plan to renew insurance for additional days coverage.
