Dear sir, I will be in the US from 8 to 15 of May together with my wife and our daughter born on dec.23 2009 (4 months old). I need health insurance for my wife and our daughter (I am calready covered by the unsurance of the company I work with). Due to my not perfect understanding of English I need a confirmation. If I fill the form for my wife and in the field "dependent children" I specify "1", does it mean that both my wife and the daughter are covered by the insurance or should I fill two separate forms one for my wife and another for my daughter? You can put your wife and one dependent children to cover your wife.
