Sample Axis Bank Deposit Slip – U.S. Visa Fee in India

The cash deposit slip at Axis Bank (and many other banks in India) has two portions that are side by side.

They are shown one below the other due to lack of enough horizontal space.

You will need to fill both the portions. They will keep the right portion. They will stamp the left portion and give it back to you. 

You don’t have to fill anything in the account number. When you write ‘US VISA APPLICATION FEES’, the bank knows what to do.
