Name: Name of the cardholder who is authorized employment. Last Name, First Name Middle Name
A#: Alien number. Anyone who is authorized to work in US will have A number. Eventually, this may be the same A# number that would appear in permanent resident card (green card).
Birthdate: Birth date of the person.
Category: Category under which the eligible person received this EAD.
Sex: Gender of the person. M/F (Male/Female).
Country of Birth: Country where the person was born in. It may be different from the country of citizenship.
Terms and Conditions: If there are any restrictions, terms, conditions; they would be mentioned here.

It mentions that 'NOT VALID FOR REENTRY INTO US'. In order to reenter in the US, you will need to have either a valid non-expired non-immigrant visa or advance parole.
