India Renunciation Certificate Documents
The following documents must be submitted to the applicable Indian Embassy/Consulate to get the surrender certificate:
- Order form:
Applicable only for the jurisdictions that processes the application through Travisa Outsourcing:
Complete the online Order Form. You must include a copy of this form with all other supporting documents.

The form must be filled in duplicate and each form must be notarized. If applying in person (wherever allowed), it should be signed in person.
- Naturalization certificate:
A copy of the Naturalization Certificate is required.
For minors who were not issued naturalization certificates, the parents must write a statement that the minor's citizenship was obtained through the parents, and provide a copy of the parents naturalization certificate.
Even though the U.S. naturalization certificate indicates that it is unlawful to make copies of that certificate, you are allowed to make a copy for lawful purposes such as for applying for a surrender certificate. You just can't make fake or fraudulent copies of that certificate.
- Most Recent Indian Passport:
Most recent Indian passport must be provided in original.
If it was lost, misplaced or submitted to foreign government earlier, the following additional documents must be submitted instead:-
Details of the last held Indian passport such as passport number, date of issue, place of issue etc.
- Notarized affidavit explaining the circumstances for loss or explaining reasons for not possessing the Indian passport.

Police report if the Indian passport was lost, stolen or misplaced.

Official documents submitted to the U.S. authority at the time of obtaining the U.S. citizenship/naturalization and I-130/I-140 and supporting documents.

Details of the last held Indian passport such as passport number, date of issue, place of issue etc.
- Copy of foreign passport:
A copy of the information page and the last two amendment pages of the U.S. or Non-Indian passport is required.

Proof of Address:
You must submit a photocopy of proof of residence/address that exactly matches the present address on the application.
You may submit a photocopy of a state issued ID (driver's license or ID), or major utility bill (water, gas, electric, sewage, land line phone). Cell phone bills, credit card statements, bank statements or lease agreements are not accepted. The address cannot include a P.O. Box.
For minors, a proof of address from both parents or a legal guardian may be submitted instead. For family (group) applications, only one copy of proof of address is required per family (group) as long as all family members reside at the same address.

Prepaid Return Shipping Label:
You must provide a prepaid shipping label along with your application so your documents can be returned to you once your application has been processed.
Do not provide an air bill using your credit card information. Travisa Outsourcing will not generate shipping labels or air bills for you and passports cannot be shipped using your credit card information.
You are strongly recommended to use a trackable method such as FedEx or UPS. You may elect to pickup your documents in person once they are ready, however you are still recommended that a prepaid air bill be provided along with the application in case your plans change.

- Other Fees
Reference Fee:
All non-U.S. passport holders must pay an additional $20 as the reference fee.
Processing Service Fee:
Those applying at the Travisa Outsourcing should add a Processing Service Fee of $12.
Fees must be paid either by money order or by certified check in the name of the Consulate General of India, <CITY> or Travisa Outsourcing (for those applying at Travisa), as appropriate. Cash, credit cards or personal checks are not accepted for mailed applications. When applying in person (wherever allowed), you can pay the fees in cash.
If you are applying for an Indian Visa and if you are sending the documents to Travisa Outsourcing, you should make it payable to 'Travisa Outsourcing' instead.
If several family members are applying through the same envelope, a combined money order/certified check may be made for the total fee of the family members. The Money order/certified check for mailing fees must be separate.
- Application Fee / Penalties:
- Write a cover letter that indicates all the documents you are enclosing. Also clearly state the circumstances
in which you acquired U.S. (or foreign) citizenship.
You must indicate the return mailing address for the cancelled passport/surrender certificate.
Not required if processed through Travisa Outsourcing.
