India Renunciation Certificate Documents
The following documents (two sets of all documents) must be submitted to get the surrender certificate:
  1. Renunciation Form
    The form you have to fill varies: The form must be filled in duplicate.

    For child above 5 years of age, signature of the minor child is mandatory. For child under the age of 5 years, thumb impression of the child is mandatory. For all minor children, signature of both the parents are mandatory.

  2. Photograph:
    One photograph, to be pasted on the application form. You don't have to provide any additional photographs.

  3. Order form:
    Applicable only for the jurisdiction that process through Cox & Kings Global Services (which is at least the entire United States):

    Complete the CKGS Order Form and print it.

  4. Additional Particulars form:
    Applicable only for the jurisdiction that processes through Cox & Kings Global Services (which is at least the entire United States):

    Complete the Additional particular form and print it.

  5. Naturalization certificate:
    Copy of naturalization certificate, as appropriate.

  6. Most Recent Indian Passport:
    Most recent Indian passport must be provided in original.

    If it was lost, misplaced or submitted to foreign government earlier, the following documents must be submitted instead:
    • Affidavit of Loss

      For those appyling in Houston, additionally,
      • a police report for the lost Indian passport.

      • either a copy of your Indian birth certificate or school leaving certificate.

    Additionally, provide the photocopy of the Indian passport, first three pages and last two pages.

  7. Copy of foreign (US or non-Indian) passport:
    A copy of the information page and the last two amendment pages of the U.S. or Non-Indian passport is required.

  8. Proof of name change

  9. Proof of address

  10. Prepaid return shipping envelope:
    Send two of them. One for returning of passport with visa, and the other for sending the Renunciation Certificate separately later.


  11. Fees:
    • Application Fee / Penalties:
      Fees and penalty charges, if applicable, as described in the rules section.

    • Other Fees

      ICWF Fee:
      All applicants need to pay $3.00 as ICWF fee.

      Reference Fee:
      All non-U.S. passport holders must pay an additional $20 as the reference fee.

      All non-US citizens residing in the US for less than 2 years and applying for any service need to pay a reference fee of $20 in addition to other fees.

      Processing Service Fee:
      Those applying at the Cox & Kings Global Servies should add a Processing Service Fee of $20.00.

      Additional charges (where applicable):
      Biometrics finger printing = $0.50
      Biometrics finger printing /Facial biometrics = $1.00

    Fees must be paid by demand draft, money order, cashier's check or banker's check in the name of the Consulate General of India, <CITY> or Cox & Kings Global Services (for those applying at Cox & Kings Global Services), as appropriate. Those applying at Cox & Kings Global Services can also pay with credit card or bank transfer. Personal checks are not accepted. When applying in person (wherever allowed), you can pay the fees in cash.

    If several family members are applying through the same envelope, a combined money order/certified check may be made for the total fee of the family members. The Money order/certified check for mailing fees must be separate.

  12. Minor applicants:
    For minors under the age of 18 years, please complete parental authorization form for minors.

  13. Write a cover letter that indicates all the documents you are enclosing. Also clearly state the circumstances in which you acquired U.S. (or foreign) citizenship. You must indicate the return mailing address for the cancelled passport/surrender certificate.

    Not required if processed through Indian Visa Application Center.
