Employment Based Greencard

I originally sent the applications back in July 23rd, but USCIS made a mistake and thought my PD was not current (which was), and returned my applications.

Here is my timeline: EB3 NSC

09/03 - Applications for (I-140, I-485) received (with PP for I-140)
09/12 - I-485 status changed to "finger print fee" received
09/13 - I-140 approved
10/07 - Fingerprint appointment notice
10/21 - Fingerprint done
10/07 - My wife's Combo card received
10/17 - My combo card received
03/03 - I-485 case status changed to "Card in production" for my wife and me
03/03 - I-485 case status changed to "Approved" (I wonder why the status changed from production to approved, which seems strange)

There has been no status change since.

Here are my few questions:
1. Are we approved and done?
2. What's my next expected case status? (Approval notice sent? or Card mailed?)
3. When can my wife leave the country and come back? (She needs to visit our country for an urgent issue)
