Affidavit for no name in birth certificate; which surname to be given?
My birth certificate does not carry my name and it just says that "female baby" I am now preparing the affidavit signed by my parents. I am struck with the last name now. Before my marriage, my last name was my father's name. after my marriage, I took my husband's name as my last name. and thats the name found in my current passport as well.
so, when I create the affidavit for my birth certificate, should my last name read as my father's name or my husband's name? Can someone help.
