I filed I-485 for my wife, and a response came back from USCIS stating:
I am confused about this response since the FRR that I submitted does contain the applicant's name and date of birth along with her family members, however in the second section there lists a person (the head of household) and has information about that person. I believe that the person viewing the document was confused and thought that the document was biographical information about the head of household (which is not the case).
Can I use a FRR as proof of Birth Certification since my wife does not have a birth certificate? And if so, can I resubmit the FRR and explain that the person at the bottom of the page is the head of household, and that the applicant's name is in the document (highlight it if necessary)? Will that suffice or will I need a different document?

"You have submitted a FCR, FRR, or a Notorial Certificate. However the evidence you submitted does not contain the applicant's name. You must submit a FCR, FRR, or a notorial certificate that contains the applicant's name. If the document is in a language other than English, you must submit a copy of the foreign document and the English translation"
