I have to get my passport renewed in USA. In my passport Surname is blank and my full name is printed on the Given Name. I have an endorsment in my passport which says, my first name should be read as 'XYZ' and Last Name should be read as 'ABC'. This was done in 2008 while I was in India. Now my passport is due for renewal. My question while filling the online passport application form, whether I should say Yes to the "Have you ever changed your Name"?
