PIO card - endorsement with new passport no

Hi,

Has anyone got a PIO card endorsement done at the Chicago consulate? I know the website says original US passports (old and new) are required when sending by mail. Just wondering if anyone has one has done this with notarized copies.

Also, if getting the endorsement done for a child and we want to go in person, is it necessary to take the child as well to the consulate?
