Name change affidavit

Hi All,
I am in the process of applying for our OCI cards. However my wife last name is different in US passport than what she has in her Indian passport.
US Passport - Married last name
Indian Passport - Maiden last name (We never got it changed in her Indian pp)

I read somewhere that if name is different b/w us and Indian passport, I need to prepare the affidavit of name change and publish the same in local paper (us and India).

Any suggestions, on what documentations that I need to prepare regarding this?
