Primary H1 rfe with Concurrent H1approved

I have been working for company A for long time and recently company B has filed concurrent H1 which was approved.
when company A filed extension, got an rfe stating that another application was approved for candidate recently and this is change of employer.
would it be a mistake on uscis...
