Can H-4 visa stamp have maiden name and passport changed to married name?

Hi,

My wife is currently on an H-4 visa. She has:

1) Her passport with maiden name
2) Her H-4 visa stamp with maiden name (valid for the next 3 years)

Now we plan to change her last name to her married name and obtain a new passport (with married name)....

I had a simple question to ask, my wife and I got married in 2014 when she already had her H1, after marriage we changed her name on passport to use my name as her middle name and my last name as hers also. Now that she is applying for H1 extension, not really sure which documents to provide...
