Marriage without Fiance visa

What is the process if a marriage takes place and the non citizen does not have a Fiance visa?

If a foreigner is in the US and marries a US citizen, they can do Adjustment of Status directly, filing I-130 and I-485 concurrently. The whole point of a K-1 fiance visa is to allow the foreigner to enter the US so they can marry and file Adjustment of Status. If the foreigner is already in the US, they can already marry and file Adjustment of Status, and K-1 is unnecessary.
