N400 is asking for Evidence of Name Chance.
Is my mother required to submit anything?
She is applying with the same name as is on her Indian passport and her Green Card.
She has used her current name since her marriage in 1963. Before than she had used her maiden name.
She does not have a birth certificate so there is no name mismatch with birth certificate. During the green card process, she had submitted a Non-availability letter from US Consulate which was accepted.
She does not have a marriage certificate. Her husband passed away 10 years ago, so she is currently widowed.
The N400 application does ask if you have ever used any other name. She does answer "Yes" and writes her maiden name here.
