H4 visa for newly born kid

Hi,

I want to apply for H4 visa for my 3 months old son in India. I am in US right now.
My wife holds valid H4 visa. I have following queries,
1) Does my wife needs to take our son to Mumbai for an interview?
2) Required documents includes, covering letter from my company. Could any one please share the sample letter with me.
3) DS160 should be filled for my son? Questions on that form really feel odd for a 3 months old kid.
