After getting my H1B approval notice I-797A, I got another I-94 attached with it. However I did not realize until I left the country (USA) that I was supposed to surrender that. I already had an I-94 (fro last time I entered US) attached to my passport which I surrendered. Now I do not know what are the consequences of this.
I have my H1B Visa interview next week. Should I mention this to someone?

I was working for my employer under OPT and was already in USA when the Visa petition was filed. I have come to India for stamping. Both the replacement I-94 (attached with I-797 notice of action) and the one attached with my passport have same serial number.
Also what is the Evidence of approved I-129? Is it I-797A that I have or is it the copy of petition that my employer filed, or is it something else?
