filling ds160 on child's behalf

My son is 3 years old, I want to fill ds-160 on his behalf for H4,also i want to fill 160 for my wife, should i answer yes /no to the question.:

1) for wife, Did anyone assist you in filling out this application? yes/no

2) for son(3 years old), Did anyone assist you in filling out this application? yes/no
