Applying for H4 visa in Dubai

I am on an H1B visa in the US. My wife wants to apply for a H4 visa from the US Consulate in Dubai (as she is a permanent resident of Dubai).
I can think of the following docs that are needed:
1. I -797
2. Copy of the petition for H1B application
3. Marriage Certificate
4. Her birth certificate.
5. Copy of my passport with H1B visa (does this need to be notarized?)

Are any additional docs needed? Has anyone applied for an H4 visa from the Dubai Consulate?
