Eligibility Fresh H1 based on Old petition

I want to know if I can use my Old H1 Petition to apply for fresh H1 without going through a fresh application
Below are my details of past stay in US

I had H1 from 2005 to 2007 and then 1st extension till 2010. I moved to India in 2009 January. Then in 2012 I got L1 visa
From 2005 to current date, Total i have had about 40 months of US stay on H1 and 36 months on L1.
Since 2012 I have been in US for 36 Months.
I have been In India continuously from 10-Jan-2009 to 20-Apr-2012.
Below are my exact entry and exit dats from US.

Wanted to know can I apply for H1 and do I have eligibility to get H1 on old petition and for how many months
